当院における直腸脱に対するメッシュを用いない手術の治療成績

Short-term outcomes of surgery for rectal prolapse

◆要旨：当院で5年間に施行した109例の直腸脱手術に対して検討を行った．9例に経会陰的手術を施行し，100例に腹腔鏡下直腸固定術を施行した．経会陰的手術の平均手術時間は68.7±43.6分，平均出血量は43.0±66.5gであった．腹腔鏡下直腸固定術の平均手術時間は145.4±39.1分，平均出血量は29.7±46.9gであった．再発率は経会陰的手術で22.2％（完全直腸脱0％，直腸粘膜脱22.2％），腹腔鏡下直腸固定術で8.0％（完全直腸脱1.0％，直腸粘膜脱7.0％）であった．どちらもこれまでの報告と比較して遜色なく，特に腹腔鏡手術は低い再発率と低侵襲性を兼ね備えた手術と考えられた．

　We reviewed 109 cases of rectal prolapse surgery performed at our hospital over a 5-year period. Nine patients underwent transperineal surgery and 100 patients underwent laparoscopic rectopexy. The mean operative time for transperineal surgery was 68.7±43.6 minutes, and the mean blood loss was 43.0±66.5g. The mean operative time for laparoscopic rectopexy was 145.4±39.1 minutes, and the mean blood loss was 29.7±46.9g. The relapse rate was 22.2％for transperineal surgery（0％for complete rectal prolapse and 22.2％for rectal mucosal prolapse）and 8.0％for laparoscopic rectal fusion（1.0％for complete rectal prolapse and 7.0％for rectal mucosal prolapse）. Laparoscopic rectopexy was considered to be a minimally invasive procedure with a low relapse rate.
