人工肛門造設部位を用いた単孔式腹腔鏡下人工肛門閉鎖術—バリウムによるS状結腸穿通術後の1例

Single-incision laparoscopic surgery for colostomy closure through the stoma site following Hartmann's operation for barium-induced sigmoid colon penetration

◆要旨：症例は56歳，女性．バリウムによるS状結腸穿通の診断で，当科で緊急開腹手術を施行した．術中所見ではS状結腸から腸間膜内への穿通を認め，便汁およびバリウムを漏出させないように穿孔部および腸間膜を含めて一括切除し，ハルトマン手術を行った．術後5か月目に，人工肛門造設部位を用いた単孔式腹腔鏡下手術（single incision laparoscopic surgery：以下，SILS）で人工肛門閉鎖術を施行した．術後経過は良好で，2週間で退院となった．人工肛門造設部位を用いたSILSによる人工肛門閉鎖術は症例を選択することによって安全に施行可能であり，新たな腹壁損傷がなく整容性にも優れた手術術式と考えられた．

　A 56-year old woman was diagnosed with barium-induced sigmoid colon penetration. We emergently performed Hartman's operation to avoid contaminating the peritoneal cavity with stool. Five months after the first surgery, we performed laparoscopic stoma closure by single-incision laparoscopic surgery through the stoma site. The postoperative course was favorable, and the patient was discharged 2 weeks postoperatively. Laparoscopic colostomy closure by single-incision laparoscopic surgery through the stoma site was safe and feasible and enabled us to improve the cosmesis and minimize abdominal wall injury.
