斜頸を呈したSandifer症候群に対しToupet法による腹腔鏡下噴門形成術を施行した2例

Laparoscopic Toupet fundoplication for a gastroesophageal reflux in young children combined with torticollis ; Sandifer syndrome : Report of two cases

◆要旨：Sandifer症候群は，胃食道逆流症と姿勢異常を特徴とする稀な症候群である．今回筆者らは斜頸を呈したSandifer症候群の2例に対して，腹腔鏡下噴門形成術による外科的治療を行ったため報告する．1例目は5歳男児で，斜頸とともに嘔吐を合併していたため胃食道逆流症の早期診断が可能であった．一方，2例目の7歳女児は，主訴が斜頸のみであったため，診断まで約5年を要した．2例とも胃酸分泌抑制薬・消化管運動機能改善薬による保存的治療では症状が改善せず，腹腔鏡下噴門形成術（Toupet法）を施行した．1例目は術後速やかに嘔吐・斜頸は改善した．2例目は頸椎の二次性骨変化を伴っていたため，斜頸の改善に時間を要している．

　Sandifer syndrome is a rare disease combined with gastroesophageal reflux and abnormal posture. We report two cases of Sandifer syndrome, showing torticollis, underwent laparoscopic fundoplication. Case 1 presented vomiting with torticollis, and was therefore diagnosed early. However, the symptom of case 2 was only torticollis, thus the diagnosis had not been completed for about 5 years. Both cases could not be cured by drug management, and consequently underwent laparoscopic Toupet fundoplication. Case 1 recovered a few months after surgery, while the torticollis of case 2 has improved but not completely, because of secondary deformation of the cervical spine.
