腹腔鏡下広汎子宮全摘術におけるCUSAの使用経験

Laparoscopic radical hysterectomy using a Cavitron Ultrasonic Surgical Aspirator

◆要旨：婦人科手術においては，超音波吸引装置（以下，CUSA）は多くの開腹手術で使用されているものの，腹腔鏡下手術では，筆者らの検索しえた範囲では1件の報告があるのみであった．近年，腹腔鏡用CUSAが開発され，5mm径のものが開発され使いやすくなった．筆者らは子宮頸癌に対する腹腔鏡下広汎子宮全摘術を26例行い，そのうち13例にCUSAを使用した．CUSAを用いた症例（CUSA群;n＝13）とCUSAを用いなかった症例（非CUSA群;n＝13）とを後方視的に比較した．CUSA群は非CUSA群に比べて有意に出血量（p＝0.010）や手術時間（p＝0.001）を減少させ，また新たに有害事象を生じることはなかった．今後症例を積み重ねて婦人科腹腔鏡下手術におけるCUSAの有用性について前方視的に検討する予定である．

　Although the Cavitron ultrasonic surgical aspirator （CUSA） is widely used for open gynecological surgery, we have found only one study reporting its use in laparoscopic surgery. Recently, a CUSA has been developed for laparoscopic surgery, which has a diameter of 5mm and is easy to use compared to other CUSAs. In this study, we performed total laparoscopic radical hysterectomy on 26 patients with cervical cancer. The patients were classified into two groups: CUSA group （n ＝ 13） and non-CUSA group （n ＝ 13）. We observed a significant decrease in the amount of bleeding （p ＝ 0.010） and surgical duration （p ＝ 0.001） in the CUSA group compared to the non-CUSA group. In addition, no new adverse events occurred in the CUSA group. We plan to collect data on more cases and prospectively investigate the utility of CUSA for total laparoscopic gynecological surgery.
