80歳以上の高齢者における腹腔鏡下胃全摘術の安全性に関する検討

Laparoscopic total gastrectomy for elderly patients with gastric cancer

◆要旨：80歳以上の高齢者胃癌患者に対する腹腔鏡下胃全摘術の安全性について検討した．2006年1月〜2014年7月に胃癌に対して腹腔鏡下胃全摘術を施行した症例を対象に，80歳以上の31例（高齢者群）と80歳未満の158例（コントロール群）に分けて患者背景，手術成績，術後早期合併症を比較検討した．腹腔内合併症の頻度は，高齢者群とコントロール群の間で有意な差は認めなかった．全身合併症は，高齢者群で術後肺炎が12.9％と有意に多く（P＝0.02），年齢が独立したリスク因子であった．筆者らは高齢者胃癌に対して腹腔鏡下胃全摘術を積極的に導入しているが，80歳以上の高齢者では術後の呼吸器合併症の頻度が高いことを念頭に置く必要がある．

We investigated the safety of laparoscopic total gastrectomy for elderly patients over 80 years of age with gastric cancer. Between January 2006 and July 2014, a total of 189 patients underwent LTG ; 31 patients were over 80 and 158 patients below 80. We evaluated the patients' background, operative findings, and postoperative complications. Postoperative complications were observed in 25.8％ in the elderly group and 17.1％ in the control group. On the other hand, respiratory complication was observed in 12.9％ in the elderly group, and age was an independent risk factor. We positively introduced LTG for elderly patients with gastric cancer, but high incidence of pneumonia should be taken into account.
