腹腔鏡下胃全摘術における食道空腸吻合トラブルに対し経口アンビルを用いて再吻合した1例

Successful re-reconstruction of esophagojejunostomy during laparoscopic total gastrectomy

◆要旨：患者は81歳，女性．貧血に対する精査目的の上部消化管内視鏡検査にて，胃癌およびMALTリンパ腫と診断され，腹腔鏡下胃全摘術を施行した．再建における食道空腸吻合はlinear staplerを用いた機能的端々吻合法にて行ったが，挙上空腸の虚血を認めたため再吻合を余儀なくされた．再吻合は経口アンビルを用いたcircular staplerによる吻合法で行い，開腹移行することなく腹腔鏡下で完遂しえた．術後は，合併症なく経過した．腹腔鏡下胃全摘術を安全に行うためには，吻合トラブルに対処できるように，様々な吻合法を習得する必要があると考えられた．

We herein report a case of successful re-reconstruction of intracorporeal esophagojejunostomy completed with a circular stapler using the transorally inserted anvil system. This procedure was performed after the failure of a functional end-to-end anastomosis that occurred during laparoscopic total gastrectomy to treat early gastric cancer. It is necessary for laparoscopic surgeons to learn several methods of anastomosis to manage the perioperative troubles that occur during reconstruction.
