ガットクランパーを用いた腹腔鏡下直腸低位前方切除術

Laparoscopic low anterior resection of rectum using GUT-CLAMPER

◆要旨：腹腔鏡下直腸低位前方切除術において，直腸の切離と吻合は，手技的に最も高難度の過程のひとつである．吻合部の再発を避けるため，直腸の切離に先立ち直腸をクランプし，洗浄を行うことが推奨されているが，クランプ方法として定まったものはいまだない．筆者らは直腸クランプにガットクランパーを使用し，良好な成績を得ている．ガットクランパーは，2本の金属バーにより3段階に分けて直腸をクランプするもので，下部直腸を腸管軸と直交方向に，確実かつ容易にクランプすることが可能である．自動縫合器の挿入や直腸切離などの操作に際しても，クランプがずれることはほとんどない．ガットクランパーの実際の使用法，および使用成績について報告する．

　In laparoscopic low anterior resection of the rectum, transection and anastomosis of the rectum are one of the most important and difficult procedures. Rectal irrigation using intestinal clamp prior to rectal transection is generally recommended in order to avoid the tumor implantation. Although many methods of clamp are reported, there is still no standard way. We used GUT-CLAMPER （Kobe Biomedics Co.Ltd, Hyogo, Japan） for rectal occlusion and resulted in good outcome. With GUT-CLAMPER, the rectum is clamped in three steps with two metallic bars. Even in the case of lower rectum, rectal occlusion and clamping in the direction perpendicular to the axis of the intestine is easy and secure. Slipping of the clamp rarely occurs while transecting the rectum using linear stapler. We now report the details and result of laparoscopic low anterior resection using GUT-CLAMPER.
