ヘルニア分類からみた鼠径部ヘルニアに対するtotally extraperitoneal preperitoneal repair（TEPP）法の有用性―再発予防の観点から

The usefulness of TEPP（totally extraperitoneal preperitoneal repair）for the exact evaluation of hernia based on hernia classification

◆要旨：鼠径部ヘルニアの分類には，nyhus分類などがあるが，術前の正確な診断が困難，あるいは分類自体が複雑であったこともあり，あまり頻用されていなかった．2006年に日本ヘルニア研究会より術中所見に基づいた鼠径部ヘルニアの分類（以下，ヘルニア分類）が提唱され，より客観的な評価ができるようになった．しかしながらこのヘルニア分類に基づいた正確な評価に関する報告はいまだ少ない．今回，筆者らはヘルニア分類からみた正確なヘルニア分類のためのtotally extraperitoneal preperitoneal repair法（TEPP法）の有用性について，前方アプローチ法と比較検討した．対象は2004年1月～2008年12月までに当科で鼠径部ヘルニアに対して手術を施行した347例（373病変）．TEPP法ではⅣ型（併存型）の頻度が有意に高く，併存型の存在診断に有用な術式と思われた．このことは，再発率の低下にもつながることが示唆された．

　The classification of the inguinal hernia was published from the Japanese Hernia Society in 2006, and performing more objective evaluation has become possible in this field. However, there are still few reports of the exact evaluation based on this hernia classification.

Concerning the usefulness of the totally extraperitoneal preperitoneal repair（TEPP）for the exact hernia classification, we performed a comparison study with the anterior approaching repair（AAR）. Three hundred and forty seven patients（373 pathological changes）underwent the operation for inguinal hernia from January 2004 to December 2008 in our department. As compared with the AAR，TEPP showed significantly high frequency of type IV（Coexistence type）. As TEPP was considered to be a useful method for the diagnosis of typeⅣ, it was suggested that TEPP may contribute to the decrease of recurrence rate for inguinal hernia operation.
