膀胱ヘルニアを合併した両側鼠径ヘルニアに対してTEPP（totally extraperitoneal preperitoneal repair）法を施行した1例

Totally extraperitoneal preperitoneal repair (TEPP) for bilateral inguinal hernias with a bladder hernia

　患者は62歳，男性．下腹部違和感を主訴に来院した．立位では両側鼠径部に膨隆を認め，臥位では消失した．CT検査では，腹直筋の両外側部に前方へ突出する軟部陰影がみられ，右側では膀胱壁の一部がその内部に入り込んでいた．立位膀胱造影検査では右鼠径部に向かって膀胱の一部が脱出している像がみられた．以上より，右側では膀胱が脱出した両側鼠径ヘルニアと診断して，TEPP（totally extraperitoneal preperitoneal repair）法による根治手術を施行した．術後経過は良好であった．術後のCT検査，立位膀胱造影検査では，両側鼠径部の軟部陰影は消失して膀胱の脱出は整復されていた．本症例の治療法としてTEPP法は安全性，根治性，整容性の点で合理的な術式と考えられた．

A 62-year-old man with lower abdominal discomfort was admitted to the hospital. Physical examination re-vealed soft bilateral inguinal masses that appeared in standing position and disappeared in supine position. CT scan revealed bilateral prominent soft density masses, the right of which contained a part of the urinary bladder. Cystography in standing position showed a partial protrusion of the bladder. The patient was diagnosed as hav-ing bilateral inguinal hernias with bladder hernia on the right side.
