小児腹部急性疾患に対する腹腔鏡下手術

Laparoscopic Surgery in Pediatric Abdominal Emergencies

　小児の腹部急性疾患に対する腹腔鏡下手術は，成人外科領域ほど汎用されていないものの，原因のはっきりしない腹痛症の病因精査や治療において，その有用性が報告されており，急性虫垂炎疑診例に対して最も汎用されている．腹腔鏡は腹腔内を広く精査でき，腹腔内癒着性病変，卵巣嚢腫捻転，内性器病変，メッケル憩室炎などとの鑑別・治療に有用とされている．腹部外傷については，CTなどの画像診断によっても臓器障害が確定できない場合に，診断目的や治療方針決定のために用いられているのが現状である．今後の小児内視鏡下手術の進歩により，外傷例においても腹腔鏡下手術が増加してくるものと考えられる．

Even though the laparoscopic surgery is not so frequently applied in the pediatric abdominal emergencies as in adult cases, its usefulness in diagnosis and treatment of ill-defined abdominal pain, especially in the suspicious cases of appendicitis, has been recently reported. Abdominal cavity could be examined more widely and thoroughly in laparoscopic surgery that in small-incision open surgery. Consequently, the intraperitoneal disease such as adhesive change, torsion of the ovary, and Meckel's diverticulum could be diagnosed correctly and managed properly.
