腎・副腎に対する経後腹膜的アプローチ法

How to approach kidney or adrenal gland in retroperitoneoscopic surgery?

　腎・副腎に対する後腹膜鏡下手術に用いられる2種類のアプローチ法を，手術手技と適応疾患あるいは手術法について考察した．副腎皮質腺腫に対する腹腔鏡下手術と後腹膜鏡下手術との比較から，後者は手術時間が約1時間短縮され，腹腔内臓器を侵すことがなく，かつ腹腔鏡下手術の持つ低侵襲性を維持している点で，将来は後腹膜臓器の大部分の良性疾患に適応が拡大されるものと考えられる．しかし，後腹膜腔気腹に関する生理学的影響はいまだ十分に安全性が確立されたものとはいえず，今後なお慎重に分析，検討すべきである．

Two different retroperitoneoscopic approaches (flank and posterior) to the kidney and adrenals are described, and the technical advantage and drawback in each approach are discussed. The clinical results of laparo-scopic (transperitoneal) and retroperitoneoscopic (extraperitoneal) adrenalectomy for adrenal cortical adenoma showed a remarkable decrease in operating time by about one hour in the latter, as compared to the former group, while the advantages of a minimally invasive surgery were well maintained. This extraperi-toneal approach can avoid unnecessary adhesion or injury of the intra-peritoneal organs.
