21．ICU多職種連携の構築—その理論と当院ICUにおける多職種連携の実際

Building interprofessional collaboration in the ICU

ICUでの診療には多職種がかかわっており，適切な多職種連携によって患者アウトカムの改善，コスト削減，患者安全の向上が得られる。多職種連携アプローチが求められる主な場面として，ICU回診，ABCDEFバンドル，ICUプロトコル，終末期ケアを例に挙げ，Donabedianモデルに準じてより効果的な連携の方法を検討した。蓄積されたエビデンスが現場で実行されるためには，現場の状況を加味しシステムと人間の両方に介入する多面的な手法が求められる。よりよい多職種連携を促進するために，チームワークやコミュニケーションなどのICU文化を醸成する必要がある。

Critical care provided in the ICU requires multidisciplinary involvement. Appropriate multidisciplinary collaboration improves patient outcomes, decreases costs and enhances medical safety. The main methods to utilize a multidisciplinary approach include ICU rounds, ABCDEF bundles, ICU protocols, and end-of-life care. This article describes effective multidisciplinary collaboration methods according to the Donabedian model. For accumulated evidence to be implemented in clinical practice, multifaceted approaches that consider on-site conditions and intervene with both systems and people are required. It is necessary to develop an ICU culture, including teamwork and communication, to promote better multidisciplinary collaboration.
