From a lot of bitter experiences with clinical trials, I suggest the key to success in clinical trials. Timing is important in clinical trials. Before the trial, a systematic review will allow one to clarify unsolved problems, and the key paper will be a good reference for the protocol. The protocol is most important and should be elaborated many times. Obtain advice from a statistician starting at the research planning stage. A strong and dedicated leader is essential for research. Present plans at academic meetings and JSEPTIC-CTG, etc., and convene the trial participants. In large-scale studies, sub-group analyses can be performed, and benefits will be set according to the number of registered cases. Hold frequent meetings and maintain contact with participants and inform them of the numbers of cumulative and new registrations. Be sure to write the results of research as a dissertation, build a relationship of trust, and attract many comrades. RCTs are much tougher than cohort studies. After achieving the above, it is necessary to establish a system that can obtain consent at all times, or to use a protocol that does not require prompt registration.
