臓器移植にはドナー・レシピエントの適応基準，適応基準の拡大，手術手技，組織適合性，免疫抑制薬と血中濃度測定による適正な投与量設計，怠薬，免疫抑制法と拒絶反応，ドナーが有する抗原に対する抗体（ドナー特異的抗体），移植関連血栓性微小血管症，感染症，長期生着と高齢化，悪性腫瘍，提供臓器不足，臓器提供者・家族のケア，倫理，そして腎移植における透析療法を経ない移植（先行的腎移植），など多数の課題がある。本稿では，移植周術期管理として重要な，移植免疫の概念と各種免疫抑制薬の解説，拒絶反応の診断と治療，感染症予防，さらに栄養管理について記載する。

There are many important issues in organ transplantation such as preparation of the donor and recipient, details of the surgical procedure, histocompatibility, malignant tumors, organ shortages, ethics, and others. Important topics related to perioperative management in transplatation including immunosuppressive agents and therapy, rejection and donor specific antibodies, and transplant-associated infectious diseases are described in this article.
