Unlike tracheal intubation for patients undergoing elective surgery in the operating room, emergency intubation of the critically ill patients has a high risk of adverse events. The most important point is to avoid lethal hypoxemia and hypotension, and therefore adequate advance preparations are necessary. Although preoxygenation and apneic oxygenation have been used to prevent hypoxemia during and after tracheal intubation, evidence for the use of these procedures in critically ill patients remains insufficient. Ketamine and fentanyl have been suggested as favorable induction agents when intubating hemodynamically unstable patients.
