3．ICUにおける気管挿管前評価と準備—適切なリスク評価と準備，チームアプローチで安全な気管挿管を目指す

Airway assessment and preparation for tracheal intubation for patients in the intensive care unit

気管挿管は重症患者の気道確保，呼吸管理のうえで，欠かせない手技である。手技の失敗は患者の予後に直結し，低酸素性脳障害や死亡など重篤な合併症を起こすため，成功率を高めるための系統立った事前の評価と準備が必要である。現在までに公開されているガイドラインは複数あり，麻酔科医向けの手術室での気管挿管，挿管困難を想定したガイドライン^1〜7）が多くを占める。重症患者を対象としたものとしては，英国のDifficult Airway Societyのガイドラインがある^1）。これらのガイドラインでは，対象とする状況設定（術者の専門，技術の習熟度，場所，対象患者）が統一されておらず，気道困難（換気困難，挿管困難，声門上デバイスの留置困難，外科的気道確保困難）の定義も一致していない。このため，本稿で対象とする日本のICUでの気管挿管前の評価と準備については，エビデンスレベルの高い内容は提示しにくいのが実情である。本稿では，可能なかぎり学術的な論拠に基づきつつ，本邦で行われているプロトコルも紹介しながら，実践的な内容となるように努める。是非それぞれの施設での今後の参考にしていただきたい。

Tracheal intubation is an essential technique for airway and respiratory management in critically ill patients. Failure of the procedure directly affects the prognosis of the patient and can lead to serious complications such as hypoxic brain injury and death. Systematic evaluation and preparation are needed to improve the success rate. Although there are several published guidelines, many are concerned with tracheal intubation in the operating room. The guideline of the Difficult Airway Society is intended for the care of critically ill patients. Since these guidelines do not have universal definitions of the difficult airway and the setting is different, it is difficult to apply them to airway assessment and preparation for tracheal intubation in a Japanese intensive care unit. We present a practical approach while introducing current guidelines and protocols.
