Tracheal intubation is an essential technique for airway and respiratory management in critically ill patients. Failure of the procedure directly affects the prognosis of the patient and can lead to serious complications such as hypoxic brain injury and death. Systematic evaluation and preparation are needed to improve the success rate. Although there are several published guidelines, many are concerned with tracheal intubation in the operating room. The guideline of the Difficult Airway Society is intended for the care of critically ill patients. Since these guidelines do not have universal definitions of the difficult airway and the setting is different, it is difficult to apply them to airway assessment and preparation for tracheal intubation in a Japanese intensive care unit. We present a practical approach while introducing current guidelines and protocols.
