The optimal nutrition support for patients with Acute Kidney Injury (AKI) is an open issue. AKI often complicates underlying conditions and multiple organ failure. The need for nutrition support should be determined mainly by the nature of the underlying disease. Daily recommended energy requirements are between 25 and 35kcal/kg,and indirect calorimetry is recommended to match actual needs. At least 1.5g/kg of protein is needed daily with additional protein to compensate for losses. In particular,patients undergoing continuous renal replacement therapy have more protein losses. Conventional formulas to predict nitrogen balance and rapid protein turnover are not precise enough to estimate actual needs in these patients. To assess these patients,a subjective global assessment with careful inspection is essential.
