Rheumatic diseases are grouped into three categories:arthritis syndrome, vasculitis syndrome, and antinuclear antibody related syndrome. Undiagnosed rheumatic disease may manifest itself as severe organ damage requiring intensive care. Some underlying diseases may be easily identified by characteristic manifestations, e.g. pulmo-renal syndrome and progressive interstitial lung disease, but others may be difficult to find a clue to establish the diagnosis. For instance, when standard therapy fails for a patient with suspected septic shock who presented with fever and hypotension with rapidly progressive limb ischemia, a diagnostic challenge is posed to consider rheumatic diseases including catastrophic antiphospholipid syndrome. In this article, we review organ specific manifestations of rheumatic diseases in the ICU. We discuss the differential diagnosis of rheumatic diseases, classified into arthritis, vasculitis and antinuclear antibody related syndrome. Focusing on underappreciated clinical presentations of rare diseases may help intensivists identify these conditions.
