An overview of mechanical ventilators

現在，人工呼吸器はさまざまなメーカーから多くの機種が販売されている。メーカーにより独自の表記や特徴がある。そのため，患者ごとの人工呼吸器設定を語るとき，同一の機種を使用している者同士では，会話は簡単に成立するが，機種が異なるとモードやパラメータの呼称が異なり，会話がかみ合わず混乱をまねくという状況がある。このような状況を受け，ユーザーからは呼称表記や単位の統一を熱望する声が常に存在しているが，いまだ解決していない。そこで本稿では，日本で使用されている主な人工呼吸器の換気モードやパラメータの呼称・用語を一覧表に示し，機械的な特徴や機能，特殊モードについてもまとめた。

●スタンバイ機能の設定や解除，above PEEPの理解は医療事故を回避するために必要である。

●BIPAP，APRVはメーカーによって独自のモード表記があり，設定方法によって異なるモードの動作を模擬できる機種がある。

●デュアル制御，PRVCなど，設定項目自体がPSVやPCVといった一般的なモードと異なり，動作の詳細が確認しにくいモードも多数存在する。

●PAV，NAVA，ASVなど独自モードが搭載された機種も増加している。

There are many manufacturers and models of mechanical ventilators. Different nomenclature may be used depending on the country and manufacturer of origin to describe a ventilator, which must be resolved by users of the ventilator. When we discuss the ventilator settings of a single ventilator at the bedside, we do not have significant trouble due to nomenclature, but when we discuss the settings used for multiple ventilators, we may have significant problems due to confusion caused by variations in nomenclature. In this review, the modes and settings of ventilators used in Japan are summarized. The mechanical features, functions and special modes, if any, of each ventilator are discussed.
