頭痛を主訴に脳神経内科を受診する患者は多い。二次性頭痛の除外と一次性頭痛，特に片頭痛を正しく診断することが重要である。片頭痛の診断には生活の支障，動作による悪化，悪心，光過敏などがポイントとなる。カルシトニン遺伝子関連ペプチド（CGRP）関連抗体薬，CGRP受容体拮抗薬が上市され，片頭痛治療の成績が格段に向上している。最適な治療を導入するためには，片頭痛の適切な診断が必須である。群発頭痛とその類縁疾患が三叉神経・自律神経性頭痛として整理されている。

Abstract

Headache is one of the most common complaints encountered by neurologists. Therefore, it is important to rule out secondary headaches and correctly diagnose primary headaches, particularly migraines. The key features for the diagnosis of migraine are disturbances in daily life, aggravation due to physical movement, nausea, and light sensitivity. CGRP-related antibodies and CGRP-receptor antagonists have been introduced in Japan. These new medications have drastically improved migraine management outcomes. Appropriate diagnosis of migraine is essential to achieve optimal treatment. Cluster headache and related diseases are classified as trigeminal autonomic cephalalgias (TACs).
