整形外科診療所が高齢者にとって果たしていた役割とその背景—高齢者の「居がい」を支える保健医療の場と看護を求めて

The Background and the Functions of the Orthopedic Clinic for Elderly Patients in Urban Area: the place for medical and nursing care supporting “IGAI” (feeling to exist) among elderly

はじめに

　他国に類をみない速さで高齢化が進んでいるわが国では，医療保険制度に加えて介護保険制度が開始され，高齢社会の保健医療福祉制度の充実を目指している。しかし，いまだ予防的な活動，あるいは高齢者が本来持っている健康を維持するセルフケア能力を支援する具体策については十分検討されているとはいいがたいのではないだろうか。高齢者たちにとって検討されるべきは，要介護になってから恩恵が受けられるサービスだけでなく，自立生活を支援するサービスなのである。

　そこで本研究は，新たな高齢者のセルフケア支援のシステムを今後検討していく上での示唆を得るものとして，かねてより筆者が行なってきた地域診療所の果たす機能と看護に関する研究を通して，高齢者が自らの健康生活を維持する上で活用してきた地域診療所の場の意味と，そこで提供されるべきこれからのケアサービスと看護のありようを検討したいと考える。

　The purpose of this study was to clarify why elderly patients Visit their favorite clinic. The data was collected at the one of orthopedic clinic in Tokyo. Twenty-one patients were interviewed and observed. Grounded Theory was used for data, analysis.

　The six reasons of their visiting were clarilied. They want to 1) take medical treatments, 2) have physician's advice, about short term goal, 3) have a sense of security in a case of emergency, 4) visit, the clinic as daily tusk, 5) get new/useful information. and 6) share their time/place with other patients.

　Through choosing the service. human relationship, and other environments, they developed their, autonomy. The autonomy includes 2 categories : 1) to take action to be existence in the community, 2) to accept their aging.

　The meaning of their visiting clinic was related to confirm and assure their existence named “IGAI”.
