過敏性肺炎の免疫学的検査

Immunological examinations in hypersensitivity pneumonitis

過敏性肺炎は環境中の抗原〔動物由来の蛋白(鳥排泄物など)，真菌/細菌，あるいは無機物(イソシアネートなど)〕が原因のアレルギー性間質性肺炎である．したがって，その診断には免疫学的検査が必要である．病歴，身体所見および画像所見から本症の可能性がある場合には，特異抗体，リンパ球刺激試験，吸入誘発試験を行う．特異抗体検査は簡便な検査で，急性症例では感度特異度も十分で診断に有用である．しかし，慢性症例では感度は不十分でリンパ球刺激試験や吸入誘発試験の結果を合わせて診断する必要がある．

HP (hypersensitivity pneumonitis) is an allergic interstitial pneumonia induced by animal proteins, fungi/bacteria or inorganic dust.　Patients with suspected HP should undergo immunological examinations including specific-antibody, LST (lymphocyte stimulation test) and IPT (inhalation provocation test).　The specific-antibody test has high sensitivity and specificity in patients with acute HP, but it is insufficient in patients with chronic HP.　When we see patients with chronic HP, we should consider the results of LST and IPT.
