認知症の分類と診断の概略

Diagnosis, differential diagnosis and nosologic classification of dementia

認知症の診断基準は，記憶障害に加えて，実行機能障害など，もう一つの認知カテゴリーを加えた複数の認知機能の障害があり，そのために，以前の機能レベルから著しく低下し，社会生活や職業遂行に重大な障害が起こる状況である．認知症に類似した病態として，せん妄やうつ状態があり，認知症の診断は，認知症であることの診断とその原因疾患の診断からなる．本稿では，認知症であることの診断と，認知症の原因疾患の分類，そして，代表的な原因疾患を概説した．

There are many possible etiologies of dementia.　Some clinical aspects of the diagnosis, differential diagnosis, standardized clinical criteria, and the nosologic classification of the dementia syndrome are presented.　In discussing the definition of the dementia syndrome and its differentation from other organic brain syndromes, the criteria of DSM-IV-TR are taken into account.　Considering the nosological classification of the dementia syndrome particular emphasis is placed on the most frequent forms of the disease, primary degenerative dementia, vascular dementia, infectious disease, and treatable dementia including metabolic disease, vitamin deficiency and normal pressure hydrocephalus.
