副腎皮質ステロイドは，現在でも多くの自己免疫疾患の標準的治療薬であるが，稀少疾患も多いため臨床的エビデンスは十分とは言えない．一般には，重症臓器障害を有するまたは予想される例には高用量，それ以外の例には低用量が使われているが，利点を示すエビデンスは必ずしも十分ではない．最近では免疫抑制薬を積極使用する傾向もあり，ステロイドは，より低用量・早めの減量が一般的となっている．また，この薬物には欠点である重篤な副作用が知られている．利点と欠点のバランスを取ることがステロイド療法では最も大切である．

Benefit and risk of glucocorticoid (GC) therapy for autoimmune diseases were reviewed.　Clinical usefulness of GC in management of autoimmune diseases has been established.　However, clinical evidences of GC therapy are still controversial due to the difficulties for conducting clinical trials for rare diseases.　In general, a high initial dose of GC is indicated to treat patients with severe organ damage, while moderate or low doses were used to manage inflammatory conditions.　Serious adverse reactions to GC were also discussed.
