In Japan, the incidence and mortality rate from stroke have markedly decreased in the past forty years due to the popularization of hypertension management, but the decline of the incidence has decelerated in recent years. The reasons for this trend are considered to be that only the incidence of lacunar infarction has decreased with time, but the incidence of atherothrombotic and cardioembolic infarctions have not, and the incidence of cerebral hemorrhage in very old persons has increased with time. Increasing metabolic disorders, such as obesity, diabetes, and dyslipidemia, and insufficient management of hypertension, particularly in old people, may contribute to these phenomena.
