糖尿病の合併症として神経障害は発生頻度が最も高く,その1つに膀胱機能障害が挙げられる.実際に,糖尿病患者の26~78%が膀胱機能障害を合併することが報告されている.実験的糖尿病モデルとして汎用されるZucker-fattyラット,ZDF-fattyラット,GK(Goto-Kakizaki)ラットおよびストレプトゾトシン(STZ)投与ラットを用いた実験において,排尿機能および膀胱の薬理学的受容体量に対照ラットと比べて有意な差異が認められ,膀胱を介した排尿機能の異常が示唆されている.
Bladder dysfunction occurs frequently as a complication of diabetics. This chapter summarizes alteration in urinary function and bladder muscarinic receptors in experimental diabetic rats such as an obesity model rats, Zucker fatty rats, a type 2 diabetic model rats, Zucker diabetic fatty rats (ZDF), Goto-Kakizaki rats and streptozotocin (STZ)-induced diabetic rats. The urodynamic parameters (volume, frequency and speed of micturition) in these rats were significantly altered compared with control rats. Furthermore, the maximal number of binding sites (Bmax) for bladder [3H] NMS binding in these rats was significantly increased in the ZDF and STZ-treated rats when compared with control rats. These results indicate that there is significant urinary dysfunction with the up-regulation of bladder muscarinic receptors in ZDF rats and STZ-treated rats.
