声帯に発生したいわゆる癌肉腫

So-called Carcinosarcoma of the Vocal Cord: Report of a Case

I．はじめに

　本邦では頭頸部癌取扱い規約においていわゆる癌肉腫（so-called carcinosarcoma）を扁平上皮癌の亜型（variant）と記載し，癌肉腫という直接的表現を避けている。それは本腫瘍の大部分を構成する間質部に対する病理組織学的見解がいまだ不統一であることに基づく処置と考えられる。表1に示したように間質組織の特微的な組織像から，全く同一内容の腫瘍と考えられる報告文献の中でその腫瘍に対する呼称は各報告者によりさまざまである_1〜10。このことは本腫瘍の興味の中心が肉腫様病変を構成する紡錘状細胞にあり，その起源を解明することに集中していることを物語るものである。また臨床的にも不明のことが多く悪性度や治療面をめぐっての関心が高い。

　最近私どもは声帯ポリープの臨床診断で摘出した腫瘤塊がいわゆる癌肉腫であった症例を経験した。本腫瘍の本邦における報告例は非常に少なく，臨床的に病理学的にその特徴を考察することははなはだ興味あることと思われるのでここに報告する。

　A 50-year-old male with complaint of hoarseness presented a polypoid mass located on the right vocal cord which was removed under laryngomicro surgery. The tumor was characteristically composed of squamous cell carcinoma and sarcoma-like components, apparently infiltrative spindle cell carcinoma. The spindle cells did not appear to betruely anaplastic but atypical changes of stroma cells. It is generally agreed that the tumor does not metastasize. However their cellular composition, histogenesis and their biologic course have notclearly been established, but the patient was given a course of irradiation therapy of 6,000 rad by telecobalt. He showed a good prognosis without recurrence after operation.
