Wegener肉芽腫症に対するlymphapheresisの試み

Lymphapheresis for Wegener's Granulomatosis

I．はじめに

　Wegener肉芽腫症は1931年Klinger^1）により初めて結節性動脈周囲炎の移行型として記載され，1936年Wegener^2）が呼吸器の壊死性肉芽腫，壊死性血管炎，壊死性糸球体腎炎を有する症例を"rhinogenic granuloma"として報告して以来，臨床的，病理学的に一つの独立疾患と考えられている。当初は予後不良と考えられていた本症も1966年Carringtonら^3）により限局型Wegener肉芽腫症という病態が提唱されてからは，steroid hormoneやcyclophosphamideなどの免疫抑制剤によく反応する症例の報告も多くみられる^4〜6）。しかし免疫抑制剤に対して抵抗性を示す症例の存在や薬物の副作用など問題点も多く，いまだ治療法の確立がなされていない。

　今回われわれは耳症状を主訴とし経過中に声門下狭窄をきたした症例を経験し，それに対して免疫抑制療法の一つとしてlymphapheresisを施行する機会を得たので，その経過を報告するとともに若干の文献的考察を加える次第である。

　A woman aged 41 with Wegener's granulomatosis which was previously unresponded to conventional therapy was treated with lymphapheresis. Repeated lymphapheresis produced lymphocyte depletion and clinical improvement. Fifty percent reduction in circulating lymphocytes was maintained for 19 months. There was no significant untoward effects by the lymphapheresis.

　Lymphapheresis is an immunosuppressive treatment of autoimmune disease, and its therapeutic application to Wegener's granulomatosis is also desirable.
