Waldeyer咽頭輪初発悪性リンパ腫に対する放射線治療と化学療法併用による治療成績

Combined Radiotherapy-Chemotherapy for Stage II Malignant Lymphoma of Waldeyer's Ring

I．はじめに

　Waldeyer咽頭輪初発悪性リンパ腫（Waldeyerリンパ腫）は，本邦ではnon-Hodgkinリンパ腫（NHL）の24〜28％を占め^1），その進展形式と予後はリンパ節初発悪性リンパ腫やリンパ節外臓器に初発するリンパ腫とも異なる^2）ため，独自の治療法の確立が望まれている。Waldeyerリンパ腫の治療は従来の放射線単独治療に代わって，最近では放射線治療と化学療法の併用（放射線治療化学療法併用療法）が一般的となり治療成績が向上したが^3〜6），研究者によって放射線治療と化学療法に対する比重の置き方に相異があることや，多数例についての治療成績の比較検討が行われなかったこともあり，放射線治療と化学療法を併用した最適な治療法はいまだ確立されていない。われわれは1977年以来Waldeyerリンパ腫に対して独自の放射線治療化学療法併用療法を試みてきたが^1），本稿では最近6年間のstage II Waldeyerリンパ腫20例に対する放射線治療化学療法併用療法の治療成績を述べ，自験例の解析と文献的考察からstage II Waldeyerリンパ腫の適正な治療法を検討した。

　Twenty patients with stage II malignant lymphoma of Waldeyer's ring were treated with combined radiotherapy and chemotherapy. Six of 7 patients with lymphoma localized in the upper neck were free from relapse for longer than 3 years and 7 months. In eleven of 13 patients with lymphoma extending to the lower neck the disease relapsed at the area beyond the irradiated field, and the remaining two were free from relapse. Three of 11 patients relapsed attained again complete remission by Adriamycin-based combination chemotherapy and they were free from the disease, but the remaining eight patients died of the disease. Adriamycin-based combination chemotherapy followed by radiotherapy could be the best modality in the treatment of stage II malignant lymphoma of Waldeyer's ring.
