耳硬化症に対するfenestration stapedectomy

Fenestration Stapedectomy for Clinical Otosclerosis

I．はじめに

　近年聴覚検査の精度が向上し，従来見過ごされていた軽度の中耳伝音障害や，感音難聴と誤診されていた伝音障害が確実に診断されるようになった。このような中耳伝音障害の中にはもちろん滲出性中耳炎などの炎症性疾患も含まれるが，聴覚検査の威力が特に発揮されるのは耳小骨の先天性奇形や先天性真珠腫である。またここで取上げた耳硬化症の摘発数も如実に増加しており，当科においては数年前までは年間1〜2例しか行われなかったアブミ骨手術も昨年は7例，今年も既に4例を数えている。このように耳硬化症や先天性耳小骨奇形など，アブミ骨に対する手術療法の機会はこれからも増加するものと予想される。

　本論文では，10年間に手術を行った臨床的耳硬化症23耳について，主としてその聴力成績をまとめた。特に現在行っているpiston-wireを使うfenestration stapedectomy開窓アブミ骨切除術の成績につき検討を加え，この方法の長所短所を考察した。

　Hearing results of twenty-three ears operated on for clinical otosclerosis were presented. The authors evaluated the advantages and disadvantages of fenestration stapedectomy using piston-wire prosthesis.

　The hearing improvement by using piston-wire prosthesis was comparatively good in speech frequencies, but simultaneously, sensorineural hearing loss in high frequencies was observed in some cases.

　Fenestration stapedectomy is a usefull technique to be recommended for minimizing the cochlear dysfunction by oval window manipulation.
