偽膜性扁桃炎を主徴とした伝染性単核症の1症例

A Case of Infectious Mononucleosis

　I．はじめに

　伝染性単核症（infectious mononucleosis以下IMと略す）は，EPstein-Barr virus（EBV）感染により，発熱，咽頭炎，リンパ節腫脹，異型リアパ球の増加，Paul-Bunnell反応（異好抗体）陽性を示す疾患として知られている。筆者らは今回，Paul-Bunnell反応は陽性を示さなかったが，定型的臨床所見とEBV感染を認めたことから，伝染性単核症と診断した症例を経験したので，ここに報告する。

　A twenty-year-old female with chief complaints of dysphagia and fever. On admission the patient was presented with pseudomembranous tonsillitis, hepatosplenomegaly and generalized skin rash. On laboratory findings, 29% of atypical lymphocytes and high titer (640 dilutions) of E-B viral capsid antigen IgG were found, although heterophile antibody (Paul-Bunnell tests) was negative, and infections mononucleosis was diagnosed. The patient was recovered in three weeks from the onset.
