I.緒言
白血病は全身疾患であるが,白血病と耳科学的所見については臨床的な研究と剖検体から採取した側頭骨の病理組織観察による研究があり,それぞれいくつかの報告がある。臨床的には耳科学的症状は最近重要視される傾向にある1)。近年化学療法が発達して白血病患者の長期生存に伴い,耳科学的症状の発現の機会も増加しつつあるように思われる。今回筆者らは白血病患者3症例6耳の側頭骨につき病理組織学的に研究したので,報告する。
Serial sections of 6 temporal bones taken from 3 patients with leukemia. There were no otological signs and symptoms described in their medical records. However, all temporal bones of these 3 cases showed pathological findings due to leukemia to varying degrees. The pathological changes were of leukemic infiltration, hemorrhage and bacterial infection. The infiltration was the most dominant finding especially in the dermal tissue of the external ear and the middle ear mucosa. Hemorrhage was observed in the inner ears of all cases. Three ears showed blood in the perilymph, whereas endolymphatic bleeding was seen in one ear. Hemorrhage in the facial nerve was found in 4 ears, though facial palsy was not reported in their medical records.
