銀沈着による鼻粘膜黒化症の1例

A CASE OF DEPOSITION OF SILVER PARTICLES IN THE NASAL AND NASOPHARYNGEAL MUCOUS MEMBRANE

　I．はじめに

　最近，著者らは鼻腔・上咽頭・咽頭側索の粘膜がびまん性に黒褐色を呈した，稀有かつ奇異な症例に遭遇した。臨床検査，鼻粘膜の光顕的・電顕的観察，さらにX線マイクロ・アナライザーによる解析により，粘膜の着色は銀粒子の粘膜沈着によるものであることを確認し，その原因が点鼻薬プロテイン銀の長期濫用によるものであることを明らかにした。

　プロテイン銀の副作用としては，アレルギー反応やアナフィラキシーショックが知られているが，広範囲の粘膜変色の原因となることはきわめて稀で，その機序についても知られていないので報告し，血管収縮剤を主成分とする点鼻薬の濫用による副作用とその対策について文献的考察を加えた。

　A rare case with black discoloration of the mucosa of the nasal cavity and nasopharynx was reported. The patient, 64-year-old male, complained of persistent nasal obstruction and serous rhinorrhea, which were improved by the correction of the nasal septal deflection.

　The light- and electron-microscopic study and x-ray microanalysis of the nasal mucosa revealed ; 1) degeneration of epithelial cells and metaplasia into squamous epithelium from ciliated columnar epithelium and 2) deposition of silver particles in the lamina propria and tela submucosa with marked increase of collagen fibers. The authors concluded that the deposition of silver particles caused by nebulization of silver-protein for a long time resulted in black discoloration of mucous membrane of the nasal cavity and nasopharynx.
