後鼻孔止血バルーンの鼻手術時試用経験

THE EXPERIENCE OF USlNG CHOANAL HEMOSTATIC BALLOON IN NASAL OPERATIONS

　Ⅰ．はじめに

　副鼻腔炎その他の局麻による鼻手術時，後鼻孔から局麻剤や血液が咽頭へ流下することは患者には不安を与え，術者は術中血液を喀出させるとか咽頭の吸引を余儀なくさせられるためかなり煩わしいことである。患者に苦痛を与えることなく術中患側後鼻孔にタンポンを施しておくことができれば大変に好都合であるが，従来のベロックタンポンでは装着が簡便でなく大きさの調節が自由にできない上に患者に苦痛を与える点も問題があつて，上述の目的に使用することができなかつた。最近発売されるようになつた高研製の止血バルーン（第1図）はシリコン製でピンチポンプによつて膨らませ，大きさを自由に調節することができるので患者に与える苦痛も少なく，装着も簡便で上記の目的に十分使用できると考えられた。ビンチポンプの直ぐ前には緑色の圧力インディケーターが備えてある。写真には3種のバルーンが見られるが，一番上部の扁平な型が発売されていたもので，中央の球形のものと一番下の西洋梨形の2種は著者が試作試用しているものである。

　The effectivity of the use of choanal hemostatic ballon in nasal operations is reported. The balloons were employed in 56 nasal and sinus operations with a great advantage, which were as follows:

　(1) It tended to lessen the patient's uneasiness during the operation by completely blocking the flow of blood and other medications used intranasally into the nasopharynx.

　(2) It obviated the necessity of the surgeon from suctioning the blood from the patient's mouth.

　(3) It prevented the patient from having nausea and vomitting which might be caused from swallowing the blood.

　(4) It prevented the tamponade gauze from dropping into the nasopharynx.

　(5) It aided to increase the reliability of measuring the blood loss.

　(6) It relieved the surgeon's anxiety from the possibility of deglutition of the blood even if the patient fell asleep.
