頭頸部悪性リンパ腫の問題点—18年間の分析

SOME PROBLEMS OF THE HEAD AND NECK MALIGNANT LYMPHOMAS: ANALYSES OF EIGHTEEN YEARS OF OBSERVATION

　Ⅰ．はじめに

　1958年3月1日から1971年5月31日までの13年間に京都府立医科大学において，悪性リンパ腫の診断のもとに初回完全照射治療をした113例中，頭頸部初発の悪性リンパ腫が94例（83.2％）もあることは，その重要性を物語つており，耳鼻咽喉科医の責任は重大である（第1表）。

　1957年1月から1975年6月30日までの18年間に，本学耳鼻咽喉科で加療した悪性リンパ腫症例は，上咽頭24例，口蓋扁桃58例，舌根部6例（以上ワルダイエル咽頭輪で88例）であり，鼻副鼻腔21例，頸部11例，頬部粘膜4例を合わすと合計124例となる。その他，中咽頭2例，軟口蓋4例，喉頭1例の7例を入れると全部で131例にもなる。そのうち1年以上経過例は118例であり，この118例について，頭頸部領域の悪性リンパ腫の問題点についていろいろ検討してみた（第2表）。

　In a series of 118 patients with head and neck malignant lymphomas treated at the Department of Otolaryngology, Kyoto Prefectural University Hospital from 1957 to June 30, 1974, statistical analyses such as five year survival rate are presented. Results derived from these analyses are as follows :

　1. The Ann Arbor Staging Classification of Hodgkin's disease is also a useful guide to the management and making prognosis of the non-Hodgkin's lymphomas.

　2. The results of cases operated for malignant lymphomas of the extranodal Waldeyer's ring were worse than those treated under radiotherapy.

　3. A new therapeutic regimen with combination of chemotherapy for stage II EA of Waldeyer's ring is proposed.

　4. Radiation dosage and the extent of the fields are discussed.

　5. The histopathologic classification proposed by Rappaport is condisidered to be recommendable.
