慢性中耳炎によるツチ骨癒着症

MALLEUS ADHESION DUE TO CHRONIC OTITIS MEDIA

　I．緒言

　慢性中耳炎患者の聴力像で，大きな気骨導差を示すにも関らず，パッチテストによる気導聴力改善がまつたく見られない場合，耳小骨の離断や固着などが考えられる。この様な症例の中には，手術時のmicroscope的観察によると，耳小骨は形態的には健常に保たれているが，ツチ骨の可動性はほとんどなく，一方アブミ骨にはある程度の可動性のみられる症例がある。この様な場合，経外耳道的にツチ骨前方で，上鼓室外側骨壁を削開するのみで，良好な耳小骨の可動性と術後聴力の改善を得る事があり，ツチ骨前方が上鼓室壁に多くは骨性に癒着した症例である。これらはツチ骨癒着症のうち，慢性中耳炎の炎症刺激による骨増殖に起因するものと思われるが，8例の自験例の経過を報告して，諸賢のご批判をあおぎたいと思う。

　Among cases of chronic otitis media, there are some in which the hearing loss will not improve by Patch test. During operation on these cases it is often found that the head of the malleus is firmly fixed devoid of movement due to osseus hypertrophy. Hearing in such cases will quite readily improve by surgical removal of the excessive bony tissues.

　There are two areas in which adhesions occur on the malleus; one, at the anterior ligamant and the other, at the lateral.

　Operative results showed improvement in all cases, except one which showed recurrence of the hearing loss probably due to recurrence of adhesion.
