誤嚥を主訴としなかつた有鉤義歯食道異物の1例

ESOPHAGEAL FOREIGN BODY DUE TO SWALLOWED PARTIAL DENTURE

　I．はじめに

　異物を誤嚥した患者はまず気管食道科（耳鼻科）か，時に外科を訪れるものである。

　無理かと思われる形や大きさのものでも自然に胃腸を通過して排出されることがあるが，特に食道壁に刺さつた異物を直達鏡下に摘出するには難易があり^1），稀に無理をして食道壁を傷つけて不幸な転帰をとつた報告がみられる。迂闊にも義歯誤嚥を主訴とせず，心窩部痛を訴えて内科に入院し，廻りくどい内科の諸検査を済ませて後に有鉤義歯食道異物と判明し，ただちに当科で直達鏡下に摘出を試みたが果さず，勝手に粥食などを飲んだところ幸か不幸か自然に胃へ脱落し，外科に依頼して胃切開により漸く義歯を取出した1例の経過を報告する。

A man, aged 51, swallowed his false tooth but not being conscious of this fact, consulted an internist who recognized the sharp leg of the false tooth imbedded in the esophageal wall at its second constriction.

An attempt at esophagoscopical removal of the foreign body was unsuccessful. Against the advice of the attending physician, the patient the following morning partook a meal of soft boiled rice and milk. Thereupon the foreign body proceeded into the stomach which had to be opened surgically for the removal.
