減衰振子様回転検査法における病的Habituation—その2．メニエール病の一考察

THE PATHOLOGIC HABITUATION IN PENDULAR ROTATION TEST. PART Ⅱ ON MÉNIÈRE'S DISEASE

　I．はじめに

　メニエール病（メ病）の平衡機能検査所見に関する報告は枚挙にいとまがない。ENGの進歩とともに回転中眼振の記録も可能となつているがフランスで主として普及している減衰振子様回転検査（PRTと略す）の原法の追試は本邦ではいまだなされていない。われわれは既にその追試結果を検討し報告してきたが^14），特に本検査法の特徴的所見として"病的habituation（P. Hと略す）"の診断的意義についての検討を行なつている際^15）P. Hを示したメ病を経験したので，これらの症例を中心にメ病の中枢性要因について検討を加えてみた。

　既にMontandon^10），Greinerら^4a）によりメ病例にも中枢性障害の際にみられるのと同様な所見，すなわち"petite écriture"（P. Eと略す），あるいは"kleine Schrift"（微小波形と仮称する。極小振幅の連続をいう。振幅5°以下，眼振数3/s以上）がみられることが報告されており，またGreinerら^4b,d）はP. Hを示した症例を記載している。

　われわれの症例はP. Eのみならず，P. Hがみられた点で興味があるばかりでなく，P. Hの観点からメ病を論じたものがみあたらないので報告する。

　The symptoms of pathologic habituation were recognized in 2 cases out of 21 Ménière's disease in whom pendular rotation test was applied. In these two cases the pendular rotation test, PRT, served as a very strong evidence in indicating the central nervous disturbance in the origin of the disease.

　Among Ménière's diseases according to the criteria, there is a possibility that those with central nervous disturbances may be included.

　It deems highly important that patients with central nervous disease or those with suspected central origin including Ménière's disease should be examined with PRT.
