喉頭蓋カルチノイド

A CARCINOID TUMOR OF THE EPIGLOTTIS

　I．はじめに

　カルチノイドの大部分は消化管^1）2），そのうちでも虫垂からもつとも多く発生するが，そのほか気管支^3）4），胆嚢^5），卵巣^6）7）などからも発生するといわれている。このたび喉頭蓋から発生したカルチノイドを経験したので報告する。喉頭から発生したカルチノイドについての文献は内外を通じて見当らない。

　A carcinoid tumor of the epiglottis, a rare situation which had not been reported in the literature heretofore, is reported. A man, aged 54, complained of a foreign body sensation in the throat at the time of his meal or whenever he attempted to swallow since 1 year previously.

　The examination showed a growth on the epiglottis the size of thumb head, relatively smooth on the surface. The growth, which measured after its removal 2.2×1.5×1.8cm and featured hard to the thouch, was surgically removed through sub-pharyngeal approach.

　Microscopic study showed the tumor cells to be mutifaceted and crowding each other resembling those of cacerous ones. The cell nuclei were round in shape and rich in chromatin granules. These cells appeared to be invading the adjacent nerve structures as well as lymphatic vessels.

　The diagnosis of carcinoid tumor was made.
