Simple Esophageal Castの1例

A SIMPLE ESOPHAGEAL EPITHELIAL CAST

　Ⅰ．緒言

　本疾患の最初の記載はBirch-Hirschfeld（1877）によつてなされたが，それ以後現在まで文献的に24例を数えるにとどまり，とくに本邦においては三浦（1955），佐野（1957）など若干の報告をみるにすぎない。本症はその発病状況が突発的かつ急激であること，治癒期間が非常に短くかつ予後も良好であることから発病の成因が明らかにされないままさまざまな推察が繰り返され今日に至つている。そこで過去に発表された症例と，われわれが経験した症例とを肉眼的，顕微鏡的に比較検討し，その成因解明に役立てたいと考えた。

　A case of esophageal cast, considered as being rare in occurrence is reported with references to other reports in the literature.

　A woman, aged 30, expectorated from the throat a tube of epithelial esophageal mucosa preceded by severe symptoms of retrosternal pain, dysphagia, nausea, excessive salivation and vomiting of fresh blood.

　A fibrescopic examination of the esophageal wall, 3 days after hospitalization of the patient, revealed a marked redness but no constrictions nor any ulceration. The examination was repeated 13 days thereafter with similar negative results.

　Microscopically the cast showed a simple epithelial cells without the subepithelial tissues infiltrated with a few leucocytic cells.
