めまい症例—小脳脳幹部Medulloblastoma

VERTIGO: MEDULLOBLASTOMA

　脳腫瘍は診察技術の向上・普及ならびに本疾患に対する関心の高まつてきたことなどよりすでに稀れな疾患ではなくなり，さらに小児の小脳虫部腫瘍Medulloblastomaも決して少ないものではない^3）4）。ここに報告する症例は頭部打撲に続いて頭痛，嘔吐，歩行障害をきたし，くも網膜下出血をうたがわれたが，耳鳴を自覚し当耳鼻科を訪れ，後に小脳脳幹部腫瘤と診断されたものである。この診断への契機として著明な特異的眼振所見があり，神経耳科学的にも興味ある症状を呈示した。本症例の経過中の眼振所見の推移を主として述べ，他の臨床検査成績ならびに剖検結果を報告し，若干の考察を加えた。

　A boy, aged 7, complained of headache, nausea, severe unilateral tinnitus aurium and disturbance of gait. Examination showed spontaneous nystagmus and an onset nystagmus at a fixation of sight. Vertical nystagmus was induced with an upward fixation of sight. A marked disturbance was noted in maintainance of body balance. The operation revealed the presence of medulloblastoma which was confirmed at the autopsy.
