鼻疾患に対する副腎皮質製剤の応用

THE EFFECT OF ADRENAL CORTEX DRUG ON NASAL DISEASES

　Ⅰ．はじめに

　副腎皮質ホルモンが耳鼻咽喉科領域に用いられるようになつてすでに数年を経ており，副腎皮質ホルモンが応用される場合がしだいに増加し，各種疾患に適用されその治療効果がつぎつぎに報告されている。また一方本剤の副作用がいろいろと論議され，多くの製剤が合成されて来た^{4）7）14）}。そのなかで副作用の少ないといわれているDexamethasoneのうち，ルセル社製デクタンを用い鼻疾患に応用した経験の一端を述べたい。

　Dexamethasoneの構造は9α-fluoro-16α-methyl predonisolone acetatで第1図のごとき構造式を有し，C₉αの位置にFがついたことC16αにCH₃がついたことで，これにより抗炎症作用を強くし，かつ副作用を少なくしている。また生理作用はHydrocortisoneの10〜数10倍といわれている。

　The result of employing adrenal cortex drug, dexamethason, for treament of various nasal affections is summarized as follows:

　1) When the agent was instilled into the sinus cavity following sinus operations, it prevented excessive formation of granulation tissues and facilitated epithelization process.

　2) When the agent was injected directly into nasal polyps it caused a complete disappearance of the growth or considerable reduction in size.

　3) The nasal allergy was markedly allayed.

　A continued use of this agent did not result in any change in the blood picture or urinary examination.
