舌甲状腺—文献の考究と1症例追加報告

　Ⅰ．舌甲状腺（Lingual thyroid）とは？

　甲状腺の原基は胎生第4週に発生，管状の発育を成し，第6週になつて始めてその下端に左右二葉の甲状腺組織を認める。然るに生後この管状組織が閉塞せず継続して存在する時舌甲状腺管（Thyroglossal duct）と称し神々の発生異常を来たすことがある。例えば甲状腺組織が下降せず舌根にとどまり舌根甲状腺を形成する。この様な非固有甲状腺（Ectopic thyroid）はそれぞれ存在する部位により命名される。特に一番発見され易い舌根甲状腺は舌根盲孔付近に位置を占め，表面光滑にして時々静脈の怒漲を見る。

　臨床上格別注意を要するは固有甲状腺を欠如した症例に於いていたずらに単なる舌腫瘍として摘出しないことである。この様に甲状腺が前頸部に無く他所に全部集中された場合を迷走甲状腺（Aberrant thyroid）と呼び舌甲状腺の70％を占める。他の30％はAccessory thyroidと呼ばれそれぞれ繊維束或いは甲状腺組織を以つて固有甲状腺と結ばれ，時には固有甲状腺と連結なしに存在する。

A case of lingual thyroid in a girl, aged 10, is reported. The diagnosis of this case was confirmed by tissue studies as well as thyroid functional test using radioactive iodine which has been recently developed.The ectopic thyroid was located at the base of the tongue close to the foramen ceacum.

This case required no treatment whatso-ever, the complaint was negligible ; the thyroid gland was absent in its usual cervical position.

A lierature review showed 161 cases of lingual thyroid reported up to 1955.
