Ida reports a case of infected tarsal cyst that presented a diagnostic difficulty because of concommittant involvement of sinusitis in the maxillary, ethmoids and left frontal sinus. The patient, affected with a fistula of the left upper eyelid evidently the result of for-mer surgical intervention, showed a marked congested swelling of the left upper eyelid with high fever and some exophthalmos. In this condition the patient arrived in the ENT Department from that of the Eye for further investigation of the sinuses as causative fact-ors of the ocular conditions. X-ray examinat ions confirmed the diagnosis of sinusitis. Con-sequently the maxillary sinus, the ethmoids and left frontal sinuses were treated by opera-tive procedures. It was only after these procedures had been undertaken that it was found that the ocular symptoms were unrelat-ed to involvements of sinusitis but, in reality due to infected unhealed tarsal cyst.
