Ⅰ．はじめに

　Malignant fibrous histiocytoma（MFH）は，四肢あるいは後腹膜といった軟部組織に好発する悪性腫瘍で，頭蓋内原発MFHは，1976年にGonzalez-Vitaleら^9）が初めて報告して以来，現在までに33例^{1-3,5,8,11,13,18,20,22,23,25,26）}が報告されている．そのうちで，脳内に発生したMFHは今回文献を渉猟した限りでは8例^{2,3,8,13,18,20,22,23）}にすぎず，組織学的に悪性転化が確認された報告はみられない．

　最近，再発時に悪性転化を来したintracerebral MFHの1手術例を経験したので，若干の文献的考察を加えて報告する．

We report a rare case of a primary intracerebral malignant fibrous histiocytoma (MFH) with malignant progression from a premalignant stage. A 73-year-old male presented with mild hemiparesis on the right side of his body 4 months prior to admission. MR images showed a homogenously enhanced mass in the left parietal lobe. The patient underwent an uneventful extir-pation of the solid mass. Microscopically, the solid tumor contained histiocytic cells and spindle-shaped fibroblastic cells arranged in a storiform pattern. The tumor cells were immunoreactive for vimentin, MAC 387 (macrophage marker) and α-1-antitrypsin.
