I．はじめに

　脊髄くも膜下腔に限局性の血塊を形成したものは脊髄くも膜下血腫と呼ばれており，原因が明らかでない特発例は過去に13例の報告があるのみである^{1,6,8,10,12-15,17,18）}．その大多数が胸椎レベルに発生し病態，治療計画，および予後に関する特異性から脊髄くも膜下出血とは別個に取り扱われてきた．

　今回われわれは，Brown-Séquard’s症候群で発症した高位頸髄背側のくも膜下血腫を経験したので文献的考察を加え報告する．

Spinal subarachnoid hematoma is a very rare event, occurring exclusively in the thoracic or lumbar re-gion. We report the first recorded case of spontaneous subarachnoid hematoma in the high cervical spine presenting as Brown-Séquard's syndrome.

A 57-year-old woman with no prior problems suddenly presented with Brown-Séquard's syndrome at the Cl - 2 disc space level following occiput neck pain. Her consciousness was clear and urinary retention was not observed. Lumbar puncture revealed no evidence of hemorrhage.
