Micro-Pressure-Suction-Irrigation Systemを併用した経鼻的下垂体腺腫摘出術

Transnasal Microsurgery Using a Micro-Pressure-Suction-Irrigation System for Pituitary Adenomas

I．はじめに

　Micro-pressure-suction-irrigation-system（MPSIS，Waldemar Link Co．，Hamburg，Germany）は，1982年ドイツ，ハンブルグ大学のLuedeckeらにより紹介された下垂体疾患専用の手術器具である^12）．Luedeckeらは，現在までに約2200例の下垂体疾患，特にその経鼻的腫瘍摘出術にMPSISを使用し，良好な結果を得ている^{2,3,8-10,13-18）}．しかし，このMPSISは現時点ではドイツの数カ所の施設で使用されているに過ぎない．今回，われわれは，この器具に改良を加え（金属タンク内に装着する日本において使用可能なinfusion bagが存在しなかったので当科で開発した．また，airpressure tubeに装着されたレギュレーターの形状を改良した），日本での使用を可能にした．1997年8月よりこのMPSISを用いて経鼻的下垂体腺腫摘出術を施行し，腫瘍摘出のみならず正常下垂体機能の温存に関しても良好な結果を得ているのでこのシステムの有用性とともに報告する．

Micro-pressure-suction-irrigation system (MPSIS), introduced by Luedecke et al, is an instrumentationfor the direct transnasal pituitary procedure. We improved this system for use in Japan. The irrigation sys-tem can effectively clean the operating field by one-hand manipulation and dissect tumor tissue by its rapidflow. The pressure of suction and irrigation can be adjusted respectively by a device in the handpiece. TheMPSIS is applicable to different stages of intervention because it is equipped with separate tips of variousdiameters, lengths and angles. This system is especially useful in combination with a micromirror or an en-closcopy for direct inspection of the eccentric tumor sites such as the cavernous sinus, the upper part of theplanum sphenoidale, or the posterior suprasellar regions. The use of the MPSIS helps to avoid injury tonormal tissue structures, and prevents tiny soft microadenoma from being lost during preparation. We haveproved the suitability and usefulness of the MPSIS in 22 surgical interventions for transnasal microsurgeryof pituitary adenomas.
