I．はじめに

　びまん性軸索損傷diffuse axonal injury（DAI）は痙性片麻痺／四肢麻痺を合併しやすい^8,11）．責任病変として，Jennetら^8）は深部白質病変を示唆し，益澤^11）は傍矢状部白質の剪断損傷（いわゆるgliding contusion）が痙性片麻痺と関連することを指摘した．われわれはDAI 100症例でCT／MRI所見を仔細に検討した結果，傍矢状部白質損傷のみならず，脳梁損傷，基底核部損傷を含めた所見が痙性片麻痺と密接に関連していることを見出した^12,13）ので報告する．

The relationship between spastic hemiplegia in dif-fuse axonal injury (DAI) and neuroradiological find-ings was studied in 100 cases. These cases were pro-spectively collected from the files of Automobile Insur-ance Rating Organization in Japan between 1993 from to 1996. Requirements for entry to this study were as follows: presence of initial unconsciousness after head injury without any lucid interval. Existence of CT scan or MRI film obtained within 12 hours of injury show-ing no significant mass effects, as well as follow-up CT scan or MRI film obtained more than 3 months after the injury.
