脳内出血で発症し急激な転帰をとった髄膜腫の1例

A Case of Meningioma Associated with Intracerebral Hemorrhage and Acute Deterioration

I．はじめに

　脳腫瘍は，一般に徐々に進行する神経脱落症状で発症し，脳卒中発作で発症することは稀である．また，脳腫瘍からの出血は転移性脳腫瘍や神経膠芽腫などの悪性脳腫瘍や下垂体腺腫ではしばしば経験されるが^50），良性腫瘍である髄膜腫からの出血は比較的少ない^8,9,18）．

　今回われわれは，脳内出血で発症し，急激な転帰をとって予後不良となった円蓋部髄膜腫の1症例を経験したので文験的考察を加えて報告する．

We report a case of convexity meningioma associated with sudden onset of intracerebral hemorrhage fol-lowed by acute deterioration.

A 76-year-old previously healthy male suddenly col-lapsed into coma and was admitted to our department, On admission, he was deeply comatose. Computerized tomography (CT) scan revealed intracerebral hemor-rhage in the right frontal lobe, together with an iso-dense mass lesion of about 5 cm in diameter. The den-sity on the fronto-medial side of the mass increased after administration of contrast medium, probably clue to extravasation. However, there was no enhancement of the mass lesion.
