Lymphocytic adenohypophysitisと考えられた1例

A Case of Pituitary Mass Suggestive of Lymphocync Adenohypophysitis

I．はじめに

　lymphocytic adenohypophysitis（Gou—die）^9）は，主に妊娠・分娩を契機として，下垂体腫瘤を形成し，出産後の経過とともに下垂体腫瘤は自然に消失し，その後に下垂体前葉機能低下を生ずるとされる．病理組織学的には，lympho，cyte, plasma cellの浸潤を主体とする下垂体前葉の慢性炎症性疾患である．この炎症による破壊によって下垂体前葉機能低下が生ずるとされている．このような病態を考えるならば，本症の非侵襲的な診断法，治療法の確立が望まれるところであり，そのnatural historyを知ることは重要と思われる．

　われわれは，病理組織は得られていないものの，臨床経過，内分泌検査よりlymphocytic adenohypophysitisと考えられた下垂体腫瘤の稀な1例を経験し，そのnatural historyを追跡することができたので，若干の文献的考察を加えて報告する．

A 29-year-old woman suffered from frontalgia with vomiting and visual fieid defects in the 9th month of pregnancy. Two weeks after delivery, a pituitary mass with suprasellar extension was demonstrated on com-puted tomography (CT). Angiogram revealed elevation of bilateral Al segments and right persistent primitive trigeminal artery. Repeated CT performed three months after delivery showed that the pituitary mass had undergone a spontaneous regression in size, and her visual impairement improved. The pituitary func-tion tests revealed no response to ACTH, GH and PRL stimulation performed about a year after delivery.
